The Federal Health Insurance Portability and Accountability Act's (HIPPA) Privacy Rule requires hospitals to have effective safeguards in place to secure the privacy of a patient's health information.

Due to these rules and regulations concerning Patient Privacy, it is most important that Parishioners notify the Parish Office (772-286-4590) if they are either going in the hospital or a local nursing home; and be sure to sign the Patient Release Form upon admittance.

If this form is not signed, the patient may be restricted outside clergy visits. Your attention to this matter is appreciated.
